To download the application form click the link "Download Form". Along with the form you should send the followings at the University Office :

1) Examination fees of Rs. 800.00 / 400.00 for SC/ST (without late fine) and Rs. 1000.00 / 500.00 for SC/ST (with late fine) is to be deposited by Demand Draft.
2)  Two copies of passport size photograph duly signed by the applicant and mentioning formno on the back. 
       
It may be noted that subject to submission of examination fees and other documents and due verification by the University, the "ADMIT CARD" may be generated and printed by the applicant.

The applicant should keep the "ADMIT CARD" with him and produce at the time of written examination.

For any future correspondence you have to refer the following information :
  a. Name of the Candidate
  b. Form No 

The last date of submission of completed forms is  February 18, 2012 (without late fine), February 25 , 2012 (with late fine) .

Please submit printed copy of the Application Form (properly filled up) along with the Demand Draft and two copies of recent passport size photograph duly signed by the applicant. Students should retain photocopies of the filled in application form and the demand draft.
